Road to hierarchical diabetes management at primary care settings in China (ROADMAP)

Aim

To establish a model of hierarchical management for diabetes in primary care settings based on the governmental guidance.

Methodology

This is a cluster randomized intervention trial. People diagnosed with Type 2 diabetes and aged from 18 to 75 years old will be recruited for this study. There will be 864 villages in 144 counties/zones of 24 provinces on the Chinese mainland enrolled and 19,008 patients (22 patients/cluster). Patients will be assessed for one year. The intervention group will receive four integrated treatment interventions.

The primary aim of this trial is a control rate of HbA1c (<7%) after one year.. Further aims include the reduction in HbA1c level, the control rate and level of blood pressure (<140/80mmHg) and LDL-C (< 2.6 mmol/L) with the proportion of patients achieving 0, 1, 2 and 3 of ABC targets at the end of study, as well as cost-effectiveness/utility ratio.
